3. กรณีชาวต่างชาติ โปรดระบุเลขประจำตัวผู้เสียภาษี 13 หลัก ที่ได้รับจากกรมสรรพากร
   In case of Foreigners, please specify a taxpayer identification number.
4. การแจ้งความประสงค์ขอยกเว้นภาษีเงินได้ มีผลกับทุกเลขที่ผู้ถือหน่วยลงทุนของกองทุนภาษีภายใต้การจัดการของบริษัท
